Reports of shortages or overages of shipments from warehouses (Form A-58, ca.1980-[ongoing].

The ABC Board is obligated to operate stores for the sale of alcololic beverages within the state. This report (Form A-58) is to be used when differences (overage or shortage) are discovered at the time a merchandize shipment is received by a store. Information on the form includes date, store number and location, invoice number, brand code, name, and quantity invoiced, brand code, name and quantity received, carrier name, and remarks. The completed form is signed by the truck driver and store manager. The original and duplicate copies are sent to the Division's Central Office and data entry personnel adjust the computer inventory records to agree with the information provided on Form A-58. The third copy is to be retained at the store.
